You need to take him to a vet to get his 12 month booster needle.
It's not the same as what pups get at 6-16 weeks old, they also need to get another vaccination at 12 months (however most trainers don't bother doing it at 12 months, they tend to wait til the pup breaks in, see if it's any good to bother spending money to get it named)

So to get your dog named, you will have to get a booster shot done first, the vet has to sign the back of the naming papers. (You don't need a vaccination card, as GRNSW only requires the vet's signature plus the sticker off the bottle which the vet will put on the paper)
